In HTML forms from CBES we are not using Submit or Reset buttons. I have updated the HTML and uploaded the same for your reference.

Checked the case with provided html for user mohd.belal@careerbuilder.com. And its working fine and able to submit the document.

Please check for the user mentioned above as I don't have credentials to Review and Sign with provided email.
