      Environment : Preproduction
      Login : Admin
      Browser : Chrome

      Steps :
      1.Login with Admin
      2.Create Document template
      3.Enter mandatory details
      4.Upload the PDF/HTML document
      5.Form fields not populating

      Expected Result : Form fields should populate to create template

      Also note, we have cleared browser cache and verified.Issue still persists.
