I attached on of their forms they use today. This is sent to the candidate via email and they have to print and fill out. We want to move to the HTML form going forward. We could have a different form per country / DOT will have their forms. Once they all get put into HTML by the devs, they will use those going forward. Let me know if you have any questions.

Hi Molly Ronovsky,

As per the latest discussion with Samir, we need to add one more field in Create Document Template as File Type to get distinction between the PDF and HTML files uploaded and manage our validations.

Please create one ticket for Aruna Raheja, so that she can update the UX for this page accordingly.

Also the HTML which you have provided above is not in a proper format and does not contains any form, can you please check and update again.

Thanks for updating the Jira for UX tickets.

While having the discussion with Swapnil Pandhare, we have 2 queries in this HTML form implementation:

1. Do we have to include to the Signature Component in these HTML forms for CBES, If YES how we need to add restriction to add Signature Component mandatory as the HTML file are directly being uploaded in Create Document page.
2. While in Preview page in _Send Form_s we currently have a restriction to make multiple page PDF Editable while using the ABCPdf service in the backend. How we will manage for combination of PDF and HTML selected by end user from Send Form page.

Also, if HTML uploaded is dependent on external(other than online links) images, css, scripts, those will not be rendered on HTML.
Also, below are few more queries :
1. Will PDF forms will also be sent from "Sent forms" page along with HTML forms?
2. In case of multiple HTML forms how the preview should be done ? Only for first HTML form?
3. When form is completed, whether thumbnail of HTML form to be included in email ?
