      Description

      Send Form Preview not loading properly if attached documents are removed and attached again.

      Steps to Reproduce:

      1. Login as Admin
      2. Go to 'Send Forms'
      3. Select any form and upload 'Action Required' and 'Reference Document'
      4. Click on Next
      5. Remove the attached document and click on cancel
      6. Select same form again and click on next.
      7. The Page doesn't load properly
