Azure - UIRefresh QA Test2 - SA Login - FormBuilder - Functional - Supervisor Approval Level Settings popup not opened if form name not given nor any message displayed.

      Description

      SA Login - Azure - UIRefresh QA Test1 - FormBuilder

      In Form Builder, While Creating a new form, if user select Supervisor Approval check box before adding form name, popup 'Supervisor Approval Level Settings' does not open. Here, at least show message such as 'Please Enter form name first' should be displayed.
