      Description

      Hi,

The Aflac Coverage Certification is missing from Additional Forms section under Employee Settings. This form is not currently visible to employees although it is set up correctly to be visible. Also, when logged into PROD you are able to see this form for Employees. Please advise as to what the issue is and provide a fix.

      Thank you,

      Kira Hamilton
      Debbie Kulling

          Hi Kira Hamilton,

We went to preprod with Partner login and edited employee. We are able to view "AFLAC Coverage Certification" menu in employee settings. Please refer below snapshot.

It may possible that employee which you edited is not eligible for additional form. It will be great help if you provide employee details for which you reported this issue.

          Thanks,
          Vijayendra

          Hi Vijayendra Shinde,

I apologize that I did not include actual Employee names initially. Below are 2 employees who are "Active" status and the classes they are in are classes that are Eligible to view the AFLAC document however, the document does not show under their Employee Settings. I've provided screenshots see NF-5852 Client Screenshots.

          Hi Kira Hamilton,

I have verified this on producation, the 'AFLAC Coverage Certification' additional form is visible for partner and company admin login but not for employee login.

'AFLAC Coverage Certification' form is inactive for employee level. Can you please verify this?

Is this required to show 'AFLAC Coverage Certification' form for eligible employee's login for both production and Pre-production branch?

          Thanks,
          Seema

I don't understand what you mean when you state that the form is "inactive" it should be viewable at the employee level

          Hi Sachin Hingole,

As per suggestion of Debbie Kulling, we made additional form menu active for employee level. This will get deployed in next build.

          Debbie Kulling,
In production and preproduction, additional form menu is inactive for employee level. Database flag IsActive decides whether to show menu to employee or not. When Isactive field is 0 this means do not show menu to employee. Vice versa when isactive is 1, then show menu to employee.
In this additional form menu case, isactive flag is set to 0. Due to this form menu is not visible for employee in Production as well as in preproduction.

As per your suggestion, we are making this menu active for employee. Now menu will be visible for eligible employees.

Let me know if you need more details on this.

          Thank you,
          Vijayendra

We have activated additional forms from the backend for EE. We checked at employee login and observed that it populated at employee logging.

I am sorry but I do not understand the explanation above. When creating a new form we have never had to do anything to have it show at both the partner/admin or employee level. The only thing that needed to be checked was the "Add form in Menu"

          What is the "IsActive" flag?

          Has something change?

          Hi Debbie Kulling,

For menu to be visible for employee, there are two customizations to be made -

          1. "Add form in Menu" check box should be checked at form builder level
          2. Newly added form should be selected in EDS screens.

If we remove form from EDS selection, form will not be visible in menu.

As shown in above image, when user remove form from EDS screen, it makes form menu inactive. So database entry in IsActive column is saved as 0.
As in production, this entry was Isactive = 0, same entry moved to preproduction during our data refresh and menu was invisible.
Now it's little difficult to know when menu became inactive in production because MenuActions do not have history to track.

We have already activated this menu in Preprod and it's visible to employee.

Please let me know if you still have any confusion about Additional form menu.

          Thanks,
          Vijayendra

          Hi Debbie Kulling,

No need to say sorry. That's my fault, I should have explained you properly. Sorry for that.

EDS screen means Employee Data Screen tab of Company Information, where we select pages which we should show in workflow. Addition and removal of additional form cause menu activation and deactivation. Please refer attached snapshot of employee data screen.

For additional for to appear in menu, we should have two customizations done-
          1. Add Form in Menu should be selected in Form builder. You are already doing that.
          2. Additional form should be in employee data screen work flow sequence. (As shown in attached image). If we remove additional form from sequence menu gets deactivated for admin and employee.

We confirmed this customization on HSPL copy of SLO and it's working fine in Preproduction.

Please let me know if I can help you anything on this.

          Thanks,
          Vijayendra
