            Hi Jennifer Leugers,

            We have run 1095C report on COB company.
            Please refer latest run '12/13/2017 10:29:11 PM' to download report.

            Observation: In this case, Name of Employer is displayed as All Status EIN on 1095-C report. Time sheet data was not available for sample employee. PFA screen shot for additional information.

            Attaching QA verification check list for additional information.
