            We have run 1095-C report on 'County of Lake' company.
            Please refer '12/14/2017 2:55:23 AM' latest run to download report.
            PFA QA verification list.

            Observation: MEC indicator is marked as NO for all 12 months in Report Analytics.

            Hi Jennifer Leugers,

            Please refer latest run '12/14/2017 10:23:53 PM'. Now in this run all valid codes are generated.
            As eligibility on this company was run not properly for previous run hence 1H & 2C codes were reflecting on data analysis report.

            Let us know, if any additional information is needed from our side.
            Observation: MEC indicator is marked as NO for all 12 months in Report Analytics.

            Jennifer Leugers and Smita Pawar Please see below final corrections needed from client:

            Just two corrections to be made!

            Jin Rodriguez needs June to be corrected to 1H in line 14 and 2D in line 16.

            Serena Smith needs January to be corrected to 1A in line 14 and 2G in line 16. 1H 2A is correct for Feb through Dec.
