Concern: EE 'MILLIE KING' is not getting rate for plan 'Spouse AD&D'

Cause: If EE is having dead record in rates table, then rate is not getting calculated with SA Utility

Correction: We have fixed code for this.

Impact Testing:
1. calculate rate having more than one dead record
2. rates table are properly terminated like 1/1/17 to 12/31/17 and then recal rate for 2018 year
3. No rate is present and recal 2018 rate

• create 2018 new rate and try out all three scenarios
• extend 2017 old rate termination date to 12/31/2018 and try out all three scenarios
