•This details our selections for individuals whose coverage remained the same for all of 2015.
• Items highlighted in yellow need to be updated on the 1095’s. (Note, we did not list out dependents in this spreadsheet. Part III dependent listing should mirror the primary insured’s listing.)
•We also listed individuals who should not be receiving a 1095 for which we received an initial 1095 (part-time employees with no health care coverage, retirees with Medicare as their primary insurance provider and retirees with no health care coverage through the Village/Library). No need to do anything here unless you believe forms for these individual needs to be distributed; We will simply disregard.
•Only 5 of the forms do not require changes; These are listed with an “ok” under the Status column.

•This details our selections for individuals who coverage experienced a change in 2015.
•Items highlighted in yellow need to be updated on the 1095’s. (Note, we only detailed dependent specifics where we felt it was necessary to update the forms correctly. When not detailed, the Part III dependent listing should mirror the primary insured’s listing.)
•There are six forms required that were not included with the initial set of 1095s provided. These are generally individuals’ whose coverage terminated in the 1st half of 2015. Details for these individuals were included in the data uploaded to EBS’s FTP site.

Please review this information and let us know if you have any questions. Additionally, we would like to receive the revised 1095’s by Monday, March 28th if at all possible so that we can complete our final review and distribute to insureds by the 03.31.16 mailing deadline. Please let us know as soon as possible if you anticipate there being any issues meeting this timing.
