I have completed analysis of Historical data sheet provided and below are the required information is missing or need your inputs.
1. "BCBS PPO" Plan having effective date 01/01/2016 is provided in Benefit Plan sheet. We will not import this plan in the system.
2. Enrollment Effective Date and Enrollment Termination Date is not provided for almost all election record. we are considering
Enrollment Effective Date = 01/01/2015
Enrollment Termination Date = 12/31/2015
Please confirm.
3. Enrollment Safe Harbor is provided only for the first election record as W2 * in benefit election sheet. Can we consider Enrollment Safe Harbor as W2 for all the remaining records in the benefit election sheet? Please confirm.*
4. Also Enrollment Employer Post-Tax and Enrollment Employee Post-Tax is provided only for the first record in the benefit election sheet.
For the remaining election data we are considering it as zero. Please confirm.
5. Enrollment Plan Design Identifier is not provided for some of the election data. Can we consider these employee waived their medical benefit?
6. Employee Eligibility Cheapest Plan is not provided in Offered Benefit sheet for some employee. We will not import these or we will ignore these records. Please Confirm.
7. Employee Eligibility Offer Start Date is not provided in Offered Benefit sheet for some employee. We will consider it as 7/1/2015.
8. Spouse Date of Birth is not provided in the Spouse Information sheet for all the spouse. Please provide this or can we consider dummy Birth date for spouse?
