There are different plans for same year before 08/01 and after 08/01 so please replace the Plan codes respectively.

I have imported attached election and offer data for Ossur company on Production.

There are 3 election records goes into bad, please find the attached bad file Bad_ElectionFile04_17_2016 11_52_19 PM.txt for more information.

I have imported offer data in POS plan. As Eligibility Credit Amount and Self only Employee Cost for offer data is not provided , I have consider both as zero and data imported .

i reviewed the updated forms and all are incorrect. Not one is correct. I have forwarded the spreadsheet with details to Nandu.

1. Delete all imported 2015 enrollment and re enrolled as per the new file
2.Employee termination date need to be updated as per the file
not only in main table but also overwrite the history data.
3. Assign the classes as per the main table for all history records.
4. check the 1095 c are coming as per the mentioned in the file.
5. if some employee has no class in the main table also then put this employee in the ossur class.

Please check the 18 complete employee forms and make sure that the Codes are coming as per the excel sheet attached above.

I checked those and agree those are correct

Adding to my comment i reviewed the attached spreadsheet and verified the dates for the 2c code as being what we need for the forms to be correct. The forms need to match this spreadsheet.

I have deleted all the enrollment data for 2015 which I had imported and re enrolled election data of attached new file. I have updated termination date and classes of employee in main and history table as well.

Also cost has not get any of the imported enrollment data. Nandkumar Prabhakar Karlekar Can you pls check this? Please find the attached election file imported on company Election File.txt .

Employee Jonathan L. Wibbenmeyer whose spouse is not exists in the system. I could not import enrollment for his spouse.

wrong codes displayed on line 16. Please refer attached excel Ossur_Observations.xls.

The January 2nd start date is incorrect it should be january 1st for all of these employees that are indicated as a Jan. 2nd start date.
