Please see attached spreadsheet for the City of Durham corrections. Please note: The client is requesting we place code 1G/2C for individual months for employees who were terminated in the prior month but still retained normal benefit coverage. All other months for each employee on the spread remains the same. The previous spreadsheet also had requested 1G/2A for individual months and dev placed the 1G code in the All 12 months box on line 14 - This is incorrect - the client wants 1G for the months identified for line 14. Please let me know if you have any questions. - Chris

NOTE - We are not showing 2C on line 16 when Line 14 has 1G, Also as per Auditors 1G and 2C code combinatinos are invalid.
IRS guidelines -
2C. Employee enrolled in coverage offered. Enter code 2C for any month in which the employee enrolled in health coverage offered by the employer for each day of the month, regardless of whether any other code in Code Series 2 (other than code 2E) might also apply (for example, the code for a section 4980H affordability safe harbor). Do not enter 2C in line 16 if code 1G is entered in the All 12 Months Box in line 14 because the employee was not a full-time employee for any months of the calendar year. Do not enter code 2C in line 16 for any month in which a terminated employee is enrolled in COBRA continuation coverage (enter code 2A).

Issue still not resolved - seeking clarification on how to proceed.
Re-opening issue as it is still not resolved.
We need some clarification on how to proceed with the remainder of the City of Durham’s 1095c corrections. Dev’s position is clear on this matter – they do not think we can use the 1G in the manner requested by the client and their broker. (see attached email). However there seems to be some gray area surrounding it’s use. I need some direction how to proceed. If we cannot use the 1G code for individual months coupled with blanks in line 16, we need an alternative for the scenario. Please advise, we need to process the remainder of the corrections ASAP.
Some context: The City’s medical coverage has a unique feature – If an employee terminates on or after the 15th of the month the employee is covered for remainder of the month as well as the entire following month. This is where the 1G comes in – the City would like to code 1G/Blank for the month of coverage after the employee terminates and then code the terminated employee 1H/2A for the remaining months of the year.
Regards,
Chris

We have done the requested correction. Now for the Employees mentioned EE will get 1G/Blank for the months mentioned in excel.
We have spot checked some of the employees.
Please refer latest run dated "6/17/2016 2:03:29 AM" of report "1095-C for Actives".
We usually give 1G to depict the coverage of non employee (Cobra or Retiree).

The requested corrections are done.
Please refer latest run dated "6/19/2016" of report "1095-C for Actives".
Please see revised corrections spreadsheet. Some unusual items.
1. M. Honeycutt - was on Durham COBRA for Jan 2015 only and then re-hired as Durham employee Feb-Dec so his coding is Jan - 1G/Blank. Feb-Dec 1A/2C. Part III (d) Covered all 12 Months - X
2. T. Knight - Enrolled in COBRA for Jan only - her coding should be Jan - 1G/Blank. Feb-Dec 1H/2A. Part III - Covered Jan only.
Spreadsheet password remains the same.

NOTE: EE Donna Long 's requested codes are 1H/2A all 12 months and Part 3 is to be marked. However we do not furnish PDF's of those EE who have 1H/2A all 12 months as it depicts that EE was not an Employee in 2015 and did not have any offer. We have not done changes to this EE.
Please review the codes and let us know.

Donna Long Employee PDF is not available as 1H/2A for all 12 months and Part 3 is to be marked. However we do not furnish PDF's of those EE who have 1H/2A all 12 months as it depicts that EE was not an Employee in 2015 and did not have any offer. We have not done changes to this EE.
Thanks,

1 left. COBRA recipient D. Long - she is not an employee but had COBRA coverage for May-Dec. I think a 1G for all 12 months will work with Part III checked for the months where she had coverage. See spreadsheet loaded today.
COBRA recipient who was not employed at any time during the year but had COBRA coverage May-Dec. I think 1G for all 12 months with just the months covered in part III checked. See attached spreadsheet. Password remains the same.
