      Description

      Below issues observed in onBoard Production deployment :
      1. When Tour Builder is selected,' Page Not Found' error displayed. PFA screen shot.
      2. When Supervisor Approval is selected, server error is displayed. PFA screen shot.

          Verified above issues on Production using company EBS exchange. Status as below :

          1. While creating new tour, message 'Form with same name already exists, please enter a different Form Name' is always displayed though form with similar name does not exists. PFA QA_11.png, QA_12.png, QA_13.png
          2. OnBoard reports are not displayed on View Report PFA QA_2.png
          5. In Form builder, on Supervisor Approval checkbox, in-complete Supervisor level setting popup opens. Due to this, user is not able to customize. This is blocking issue. PFA QA_5.png
          6. Server error on Preview Tab of Preboard tour. PFA QA_6.png

          Verified on Production using company City of lake worth for hspl. For Server error in Preview Tour tab, JIRA WT-4630 is open. Other issues are resolved.
