We have done analysis of provided sheet WORKTERRA ACAMP IRS Reporting Specifications 12 13 16.xlsx and below is our analysis details.
1. Election / Enrollment data is not provided in 'benefit Election' tab, we are assuming that there is no election data for import.
2. There is total 1070 employee is provided in Employee Information tab. Almost 1019 employee is already exists in the system.
Below required fields is missing for remaining 51 employees to import in the system.

• Employee effective date is not provided. (Should we consider Employment date as effective date?)
• Termination date is not provided for some employees.
• Employee Class1,class2 and class3 value is not provided.

3. We have found some discrepancies for existing 1019 employee, with employee data in the system and employee data provided in file.

• Some employees are Active in the employee Information tab but they are terminated in the system. Which data Should we consider as an appropriate data, system data or data provided in 'Employee Information' tab?
• For some of the terminated employees, Termination date in system and termination date in 'Employee Information' tab are different. Should we consider termination date in the system?

We recommend that, we should not change or override existing data in the system.

4. While analyzing child data information in "Dependent Information" tab, only 19 Child is not exists in the system.
We need to import these 19 child into system, but gender of children is not provided.

5. While analyzing spouse data in "Spouse Information" tab, only 11 spouse is not exists in the system.
We need to import these 11 spouse into system, but gender these spouse are not provided.

6. Two medical plans HSA Premium Medical and HSA Premium Medical Plan is already exists in the system whose effective and termination dates are
01/01/2015 and 12/31/2079 respectively.
Benefit plan tab contain plan HS.A Premium, Which plan (HSA Premium Medical and HSA Premium Medical Plan) in system should we consider as provided plan?

Please find the attached Employee_Not_Exists.xls sheet contains list of 51 employees those need to import in the system along with missing required fields. Also contain 19 dependent and 11 spouse whose gender has not provided.
Once we will get the above required information, we will import this into system.

Can we discuss this in Monday or Tuesday's ACA call?

We will start this task on Tuesday 14 Feb 2016 and need 2 days to complete the same.

Nandu - anyway this can be taken on priority? We are in total crunch time for these and have noticed most have a four to six day turnaround which just can't happen. Most of these have to be finalized next week in order to meet the mailing deadline of 3/2. What is taking priority over assisting with the form generations?

Hi Jennifer Leugers,

We have imported provided WORKTERRA ACAMP IRS Reporting Specifications 12 13 16.xlsx historical data into the system.We have also executed eligibility for all the employee, so there is no need to check "Process Employee Eligibility Data" checkbox while executing 1095C report.
You can executes 1095C for Fazolis.

In the previous file Fazoli Missing Data Elements for ACA.xlsx , you have provided termination date and classes for below 2 employees. As we don't have sufficient required information to import these employee, we have not imported them into the system. Client has to add these employee and their dependent information manually into the system.

Also for the below dependents ,we have not found their employee information in the system or not provided in WORKTERRA ACAMP IRS Reporting Specifications 12 13 16.xlsx file, so we can not import these children into the system. You need to add their employee and then add child data into the system.

Under the XKC run, there are individuals showing that are not in that class and have never been. Examples - Wyatt Anderson, Brent Bernard, Julia Forward. They are on the analysis but forms are not created. Please explain why. Thanks.

While doing part 3 corrections of mentioned employees, EIN was not provided.
So such employee's data is reflected on report.

Currently we have updated EIN's for mentioned employees [Wyatt Anderson, Brent Bernard, Julia Forward]
and Rerun the reports and verified are not reflecting on Data Analysis report.

TRAVIS INGWERSEN--Line 14 & Line 16 codes combination is invalid for month May.

Please resolve.
We are done with code correction for rest of the employees.

1.Log in -> Select company -> BenAdmin -> ACA -> ACA Customization
2.Go to ACA 1095C Corrections tab
3.Enter employee SSN and click on submit button.

Next screen you can correct codes and save.
We have uploaded demo videos for code correction on EBS FTP.

Below are FTP details. Please refer "1095CManualCorrection.mp4" video file from zip folder.
