        This excel file cannot be downloaded because of following issue:
        Number of rows exceeds the maximum possible rows per sheet in this format; Rows Requested: 70535, Max Rows: 65536

        As the rows are exceeding excel sheets's limit,it gives page not found error.
        I have extracted the data from backend and mailing you the excel sheet.

        All - not sure why we need Dave's approval on this; the file is simply getting corrupted when we try to open it. We need the forms broken down in smaller batches or need the forms individually loaded somewhere so that we can zip them into smaller files for the client.

        We did not get any corrupt file message when we downloaded zip file from site and tried to open it.

        The reason we asked for Dave's approval is because for most of the companies, we furnish around 3000 pdf's in single report.So if such type is issues occur, we need to create bandwidth on our end to handle such support task and in earlier comment exact nature of issue was not mentioned.

        1G is displayed because those employees are enrolled in self insured plan called "Medimpact".
        Medimpact plan under Medimpact benefit type is marked as ACA plan and is self insured plan. This plan is also selected on 1095C report customization.

        Hello Ramya,

        Can you look into the system configuration for El Camino Hospital due to the following issues.

        I ran out an enrollment report and compared it to the ACA analytic spreadsheet and found for the 1E_2B folks we have an active enrollments in WT for 2016. Perry Aganad and Robert Almeida being examples. For Perry Aganad- His status went from PT to FT effective 1/27/2016. The 1095 for the month of Jan/ Feb shows 2B. I would think the code for Feb should be 2D the 1095 is showing 2B for Feb and EE became eligible for benefits 03/2016.

        Also, the client reported the below issues as well:

        We also noticed that some employees coded 1E have a line 16 code of 2B and that should not be the case because if they are 1E that means coverage was offered therefore we need to specify if they elected coverage or waived. Please advise what code you would like 2B changed to? the system is reading them as a part-time so pulling the 2B. It should either be 2C (enrolled) or 2G (waived). If there is no election for them in the system it should be 2G.

        Also some with 1E have a blank on line 16 and we need to have some type of code for line 16. An example of that is Ronan Acot.

        Again – we just need to know what codes you would like for us to use on line 16? They are showing a blank as they do not satisfy the safe harbor (FPL) and since they are not enrolled, the safe harbor code of 2G would normally go there. If they are not enrolled it should be a 2G and not left blank.

        Anyone with code 1H should have line 16 code 2A, 2D or 2B. Can you confirm to see if that can be pulled from the system or do we need to tell EBS what the code is for the blanks?

        This employee is not enrolled in Jan/Feb for medical plan so 2B is coming. And this employee Type on demographic is set to PT.

        Robert Almeida:
        This employee need to pay $805 cheapest employee only tier cost and the annual salary of this employee is “83512” so populating the Safe harbor code 9.66% of employee monthly salary is greater than the 805.*

        Also, the client reported the below issues as well:

        Can you advise why we are using the Part-Time indicator on the ACA analytic spreadsheet for ECH as we don't load hours. The client requested that we update the spreadsheet to reflect the correct status in column R- For example, for Diana Barile is listed as Part-time and should be full time. Please run out a corrected file by tomorrow to reflect status that indicated on the demographic for all employees.

        Part II correction file -

        XXX58470 - Employee SSN doesn't exist in the workterra system.

        All other employees provided were imported successfully for Part II file.

        Part III Correction file issues found were -

        1) The Employee SSN and the Member SSN / DOB doesn't match.
        For example -
        Employee SSN = XXXX97511 and its given Spouse SSN i.e Member SSN = XXXX55142 which doesn't match.

        Also if you are providing Correction for Employee then its Member SSN and Employee SSN must be same
        Ex -
        Employee SSN - 123456789
        MemberType - EE
        Member SSN - 123456789

        2) Some of the Employee SSN are blank, it is mandatory to provide employee SSN.

        Due to above issues, we were unable to import Part III Correction file.
        Please provide correct Part III Correction file, so that we can import it and run then 1095C on the main company

        Note - We tested this on HSPL company.

        Hi all - the Part II file can go ahead and be loaded into our Production site. We don't need Part III to be loaded for now. I attempted to load the Part II again today and was unsuccessful. So far we haven't been able to import any bulk correction files. Are we doing something wrong?

        Hi Jennifer Leugers,

        We have imported the Part II correction file and re-run the 1095C. Please refer latest run (2/22/2017 9:56:09 PM)

        The reason why you were unable to import the correction file is -

        Part II Correction File -
        1) The file must be in the format same as attached file Sample File -Part II.xls

        Part III Correction File -

        1) The file must be in the format same as attached file Sample File -Part III.xls
        2) There was discrepany from our side that if the Correction is for Employee then its Member type in Template is set to Employee it should not be EE. Same for Spouse and child it should not be SP & CH, it should be Spouse and chlid
        We have updated the Specification template

        Note - Before importing the files it will be good if you verfiy this -

        1. Navigate to Template Management >> Import
        2. Select the template which you are importing
        3. Navigate to Data Transformation tab and select sub-template
        4. You will see Fields ex - Part 2 EIN,
        5. Click on Map field and make sure that EIN value which you are importing is same as the Map Data Column.
          For ex - If Map data for one EIN is 123456789 then value of EIN in file must be 123456789, either it will go into bad file.
          Same in-case of Part 3 Member Type Refer

Please refer latest run (2/22/2017 9:56:09 PM) The reason why you were unable to import the correction file is - Part II Correction File - 1) The file must be in the format same as attached file Sample File -Part II.xls Part III Correction File - 1) The file must be in the format same as attached file Sample File -Part III.xls 2) There was discrepany from our side that if the Correction is for Employee then its Member type in Template is set to Employee it should not be EE. Same for Spouse and child it should not be SP & CH, it should be Spouse and chlid We have updated the Specification template Note - Before importing the files it will be good if you verfiy this - Navigate to Template Management >> Import Select the template which you are importing Navigate to Data Transformation tab and select sub-template You will see Fields ex - Part 2 EIN, Click on Map field and make sure that EIN value which you are importing is same as the Map Data Column. For ex - If Map data for one EIN is 123456789 then value of EIN in file must be 123456789, either it will go into bad file.
