Hello,
Please load the attached records per the provided spreadsheet for Cobra enrollments. We do not administer Cobra nor do we have Cobra plans in Wt. I believe last year the records were loaded from the backend by creating general plans for 2016. can you do the same process as last year to get the records loaded into WT.

We have analyzed file. File contains distinct 34 employees,out of these 34 employee 16 employees are already exists in the systems. We need to import 18 employee in the system ASML_Employee_Not_Exists.xls .
1. Can you please provide dependent census report for these employees? So that we will import employee and dependent information into the system.
2. Also can you please provide expected 1095C codes for these 34 employees? Once you will provide the expected codes then we will import these codes into the system will get more accurate results.

I responded to this six days ago. Please upload the missing Cobra participants and correct the existing cobra participants termination dates per the attached spreadsheet. We really need to generate the draft forms the Cobra status folks.

1. We have done analysis of the file . we have found 6 below employee are not exists in the system. We need to import these employee into the system.

But to import these employee below required information is missing in provided sheet.
- Employee effective date .
- Employee employment Date
- Payroll frequency
- Gross annual Salary
- EmployeeType for above last 3 employee is UNKNOWN then what should be its class1 value. (FTE,PT or EX)
Please provide above mentioned information for these 6 employee then we can import these employee.
2. Medical cobra plan mentioned in sheet does not exist in the system. First we need to import these plan into the system.
- CIGNA Medical HSA EE + Dependents CA/NC
- CIGNA Medical HSA EE + Dependents non CA/NC
- CIGNA Medical HSA EE only non CA/NC
- CIGNA Medical OAP CA/NC
- CIGNA Medical OAP non CA/NC
But to import these plan into the system, need below required formation.
- a. Plan Start Date
- b. Plan End Date
3. To import medical enrollment data into the system we need Enrollment Employer Post-Tax and Enrollment Employee Post-Tax. Please provide these for all the employee.
4. To process ACA 1095C for any employee we need offer data for that employee.
what is offer for these employee.
Do they have offer of any other medical plan apart from the enrollment plan mentioned in the sheet?
Or should we consider enrollment data also as offer data for these employee?
It will better if you will provide all the data in our decided format for the historical data import WORKTERRA ACAMP IRS Reporting Specifications.xls .
Otherwise best way is as per above comment 24/Nov/16 4:54 AM, Provide expected 1095C code for these employee, We will import these codes directly into the system.
Since they people listed below are not employees (they are dependents/ survivors of employees) you can use the same data (effective date, salary, frequency) as the employee if you need something filled.

Use the same plans that are loaded similar to last year COBRA. The non CA/ NC etc is what they send to Cigna. You don’t need to build them. Similar to what is set in system now. They only receive the same information that employee elect. How was COBRA done last year when it was loaded? It would look the same. Except the coverage is 100% employee paid. COBRA rates attached?
We are in process to import provided data into the system.
1. Provided sheet contains only dependent enrollment data for the below dependents. It does not contain the employee enrollment data. Should we enrolled employee with same enrollment data provided for the dependent enrollment data (plan name,effective and termination date) into the system? We can not import only dependent enrollment data into the system

What should be termination date for this employee as termination date is not provided in the sheet.?

1. Yes, please use the dependent information for the employee record.
2. No, do not terminate the employee the record should remain active. Dependent Anouk Govil should show the termination due to overage dependent status effective 07/31/2016.
I have also attached the Cobra rates to ensure the correct cost is reflected on the 1095. ASML Rate Sheet 2016.pdf
We have imported employee and dependent information into the system and generated 1095C. Please find the attached 1095C PDF File.zip file contains 1095C for all the provided employee.
1095C PDF file for employee Steven Westervelt is not generated as this employee is terminated on 01/01/2015. As he is not active single day in 2016.

Hello Yogesh,
I have completed and attached the spreadsheet for the Cobra Participants. Keep in mind this sheet only represents when they were in COBRA. If they had coverage through ASML prior in the year it does not account for that. Please refer to WT for prior coverage.
IE: Darbisi – Employed at ASML Jan- Feb. Cobra March – June. Sheet only shows March through June since that is when he was on COBRA.

We are not able to see Line14, Line15 and Line16 codes in the above attached file for all the employees. Also in the above file, *Month Index * column should be the month value not the range of months. If you wants to import code for Jan to March months then there should be 3 rows with month Index as 01,02, and 03 respectively.
Please provide the file again with expected codes and reformat file with month index column.
IE: Darbisi – Employee is not employed from March, System has provided 1H and 2A code for Line14 and Line16 respectively.
