Hi all - multiple questions on this group.

Brian Bingman is not showing any dependents on his 1095C form.

They are also seeing numerous individuals that are not showing on the ACA 1095C Covered Individuals tab -

) The following new hires are still not on the “ACA 1095C Covered Individuals” tab, even though they have elected coverage:

· Matthew Caseres (and spouse, Jae Caseres), Kaiser HMO EE+1 effective 11/1/16)

· Shannon Fuchs (Kaiser HMO Single effective 11/1/16)

· James Chavez (Anthem PPO EE+Fam effective 12/1/16)

· Taylor Rudnick (Kaiser HMO Single effective 11/1/16)

· Joseph Ferraro (Kaiser HMO Single effective 11/1/16)

· Dana Andrews (Kaiser HMO Single effective 10/1/16)

3) Andrew Harris is still not on the “ACA 1095C Covered Individuals” tab.

Are single coverages not listed there? That would explain some but not all of the issues.

The above employees are not enrolled in Self Insured Plan, hence they are not showing under covered individuals tab.

James Chavez - We have checked that this employee is under self insured plan and is also showing under Covered individual tab.

Are single coverages not listed there?
Ans - Yes single coverages are also listed under covered individuals tab.

OCSD - ACA 1095C file 3/27 versus 4/12 run.
The 3/27 file has count of 755. 4/12 Updated 1 record (Daniel Cardona).
4/12 Ran/saved file and the count went down. This run took out all the retirees - see attached list of those retirees.
This run on 4/12 should have went up by 1 to 756, instead it went down to 683 taking out all the retirees that should be included as they are in the 3/27 run adding Daniel Cardona in.

There was discrepancy from our side in SP which we have fixed on this company and its patch will be deployed next week.
We have re-run the 1095C report and now you can see total EE count as 756

Hi, will you please look at this 1095C record. The EE:
Richard Schlegel is a retiree and his dependents are not covered but show up on this 1095C.
The Client wants a correct 1095C form.

We did analysis of mentioned issue and observed that - correction for Richard Schlegel is done through "ACA 1095C correction".In this correction Dependent also selected for all months.
Please see attached screen shot for more info.

We have removed the Covered Individuals for dependents and save the correction.
After above correction, we have tried to re-run 1095C but we could not because server CPU utilization went to high.

Kindly request you don't run this 1095C from your end. We will update you on 05/02/2017.
