Demographics Import :

Issue : Kayla O'Hara - Employee status mentioned as Terminated in File But not on UI.

That same issue might be exist for other added employee's.

Spouse Import :

Sujeev J Nair - Employee status change effective date changed from '07/01/2014' to '01/09/2015'. and Spouse termination date changed from '01/01/2016' to '01/09/2015'.

The Terminated Employees issue is solved now, and the data is corrected for missed Employees.

For Effectivedate, it was provided when importing employee but not provided while importing spouse hence assigned as 01/01/2016. As per discussion with Yogesh, its not going to affect the reports and the current effective date is correct,

The client has reported patterns of the issues that has been identified on the 1095 PDF. The attached spread lINIUM C&C 1095.zip sheet for Linium C&C Computers all EE are listed with code 1H. All these EE are full time and therefore was offered benefits. All full time EE's are offered benefits.

We need to re-run the 1095's after making the corrections to the records. Attached are patterns reported after reviewing the Linium LLC spreadsheet and 1095 provided. Please keep in mind that all full time EE's are offered benefits

A pattern I’ve seen so far:

· The EEs that were offered benefits but didn’t elect coverage are showing as ‘1H’ and/or blank. (i.e. Rachel Slater)

· The EEs that were offered benefits who elected the higher coverage are showing ‘1F’. (i.e. Elissa Silverman)

· It doesn’t look like historical data from 1/1-6/30 is reflecting on their 1095s (i.e. Evin Hill)

· It doesn’t look like dependents are reflected on the 1095s either (i.e. Evin Hill, Angelina Masini)

Lisa, from your questions above - would you look in WT and verify # 3&4 and let me know if the data is in the system

1. 3. I ran and enrollment history report using 2015 dates. I don't see the historical data loaded for EE we only have dates starting from 07/1/15.
   #4. Angelina Masini - we currently have dependents enrolled and therefore could be reflected on 1095 .
   Evin Hill- we currently have dependents enrolled and therefore could be reflected on 1095 .

Please check all companies to ensure all 1095 reflects benefits offered for all full time EE and verify that codes are reflected correctly for the specified months.

We noticed on one of the recent reports, that we have some termed EEs that are now showing active. We checked Job Diva to make sure it wasn’t an error on our end first. Denise has about 22 people impacted on her side, and we haven’t checked the other staffing agencies.

Can you verify if this is related to the ACA data load? if so, please make a correction to the affected records.

As an example, Anthony Forstner is one of the impacted EEs.

It appears we still have some data errors on the 1095's. We loaded historical data for 100 plus EE's. it appears there 1095's are not correct based on their elections. In additional, I re-ran the eligibility on 04/29 and sent the 1095's to client for verification. Using Gregory brewer as and example. the last 1095 file sent to client states who states a 1H all year and the 1095 viewable under EE record shows 1E which is correct. Why is there a discrepancy from what's showing in WT verses the 1095 viewable under EE record. can you correct?

Please change the finalize run so which you think is correct these 1095c will get display at employee level.

A.Verified Employee 'Diana Matheson' was enrolled in plan mentioned in Excel sheet and enrolled cost is also correct.

B.Other Employees Verified are as follows.

2.Lauren Pressman

Observations are as follows.
a.Employees were enrolled in plan other than mentioned in Excel sheet as it was having previous enrollment.
Hence we could not verify the cost mentioned in the sheet
b.As employee has continuous enrollment in Health plan and all medical plans are ACA plan as per current ACA
customization.
C.Verified Employee from previous load file 'David Pieczonka' was enrolled in plan mentioned as per Execel sheet.

There are 12 employee is not exists in the system. If we need to enrolled election for 9 employee out of these 12 then first we will have to import these employee in the system to get 1095C. We need demographic and enrollment data for these 9 employee from client in decided format.

Also for other point, system only validating or checking employee based on his SSN, so the first-name and last-name value had not affect on load. Data is loaded correctly.

Data provided for enrollment for many employee is already enrolled in same or other medical plan in the system. If the employee is already enrolled in any medical plan for given period then we are not importing provided enrollment. We have enrolled only few enrollment in the system as many were already exists in the system.

We have done some analysis for employee Meagan Mann.
We have raised qualifying event dated on 04/01/2015 for this employee. As employee is not eligible for any medical benefit type on enroll now screen no any eligible plan is getting displayed. Please find the attached screen shot for more more information .

I have checked in history of this employee, as per history employee is terminated from 08/14/2015 but its effective date is 01/01/2015 in history table. Thats why its showing invalid code for this .

We have verified codes on 1095-C report of above mentioned employee and are correct.

Date of hire: 01/20/2015
Date of termination: 08/14/2015

1.Received 1H and 2D from month Jan to Mar as employee is hired on 01/20/2015.

2.Employee was eligible to 'BSNENY EPO 6018' medical plan that covers all dependent from month April to July and Safe harbor
is W2. Employee has received an offer as not enrolled in plan, showing 1E and 2F code for the month April to July.

3.Employee was eligible to 'BSNENY - Bronze' plan that covers all dependent in the month of July and safe Harbor is W2.
This plan is not afforadable to employee hence only received line 14 code as 1E. [As 'EE cost' of the plan is not less than 9.8% of employee's monthly salary, line 16 code is blank.]

4.Employee was terminated on 08/14/2015 hence received 1H and 2B in the month of August.

Please force the 1095 corrections for the records listed on the spread sheet attached today.

Also, for Linium Tech Search Company Maureen O'Brien (listed on spread sheet), we should have full time EE's line 14 1A, as offers of coverage was made regardless of if EE waived offer.

Linium West- example EE Craig Granier; 1095 under Ee record coding is incorrect. However, the 1095 on the company level the last eligibly run 04/29 1095 is showing to be correct. Please finalize 1095 to show correct under EE record.

We show Part 3 marked if the employee is enrolled in self insured coverage. IRS may not accept this scenario. Please verify.

While modifying file in required file, we have following observations,

1.We have not considered terminated employees.
2.Employee name Bronchetti Thomas
received offer from Feb to Oct [codes received as 1E - 2C] & not received any offer from Nov to Dec [Codes received as 1H- 2A] but in excel sheet, it is mentioned that there is offer in the month of Nov month also.

Note:- Currently we have considered no offer in the month of Nov.

We have done the 1095C Reports corrections.
Refer attached file Linium-EmployeeVerified.txt for the verified employees 1095C PDFs after corrections, These employee PDF reflects details as per excel.

Davis Jenni employee has 2A (Terminated) for May to Sept on line16 hence line 14 shows 1H. Due to this codes on PDF are different than excel.

Goetz Karen and Levine Danielle employee are not added in Linium production, So 1095C PDFs is not generated for them

Hunt Jill employee PDF is not available under any 1095C, Because employee eligibility is different than 1095C reports eligibility rule.

This is again 1H/2C codes. I made changes for this employee like this : line 14: Jan-June 1H July - Sept 1E, line16 : Jan-June Blank July - Sept 2C.

Codes for Dada Ali is given as 1E/2C all 12 months. However costs are Jan-Sept=$187.40 and Oct-Dec=$203.
But till now,we have given Jan-Sept=$203 and Oct-Dec=$187.40.So we have not made any changes related to costs. Please confirm.

Hey guys,

We made an error. the cost on the 1095 us showing in reversed.
The $203.00 should be from January – June. From July – December, it should be $187.40.
