Overview:
Allow DMS user the ability to upload a pre-created documents to the DMS that can be stored in a library, as well as sent to an end user.

Entry:

• User enters DMS platform via link or consuming application.

Acceptance Criteria:
*IF I navigate to the Upload Form area, I am able to attach a new document to upload.
*IF i have attached a new document to upload, I have the ability to fill out the following fields so that I can properly describe my document: Name, Description, mark whether it's Private or Public (https://jira.workterra.net/projects/DMS/issues/DMS-19?filter=allissues), mark whether it's a Signature or Reference document (https://jira.workterra.net/projects/DMS/issues/DMS-18?filter=allissues).
*IF i have attached a new document to upload, I have the option to map fields from consuming applications into the document so that end users do not have to provide the same information more than once. (more to come on this).
*IF I have attached a new document to upload, the document may contain any number of pages and attributes so that I do not have any limitations.
*IF I have attached a new document to upload, I am able to indicate that an electronic signature is required.

Exit:
*DMS user saves uploaded document.
*DMS user exits the DMS platform.

Prerequisites:
*User is a current user of CBAT, CBES, or WT OR has stand alone link to log in.
*User knows how to access portal either by link or within CB application.
