As mentioned in actual screenshot, Action required and Reference document is not marked as mandatory.
So either asterisk mark of these fields should be removed or if they are mandatory then validation message should be there if the document is not uploaded and click on send.

So removing the asterisk mark over these file uploads section.

I checked the case, and it's not present in develop branch right now.

So once the pre-prod will be updated with the latest develop pull, it won't replicate.
