Description: DMS does not save the selction on check boxes nor the candidate signature

and I have tested with candidate TEST2 Xavier (candID: 800)
Please open a (high priority) JIRA card with thee DMS team.
Currently only 1 mapped signature field is supported, this template has 3 unmapped signature fields, which is causing signature to come empty.

And for checkbox, there is limitation for field value, it should be "true", currently field value for all 4 checkboxes is set to "Yes". Can you please update PDF template to have field value as "true".
