Pre-requisites :
1) Template should be created
2) User should send the form
Replication steps :
1) Email triggers on sending form
2) Click on Email link of "Review and complete"
3) Navigate to Edit document page
4) Click on send button
5) Send pop-up should be displayed.
Expected Result : Send pop-up should be displayed

Jayshree Nagpure we have change send text to submit.
When we click on submit, sometimes we are getting server error and sometime the file is sent successfully.
Can you please check the reason behind intermittent issue.
