Overview:
Allow end users to receive email notifications when they have a new document they need to complete, as well as let the DMS user know when documents have been completed or are delinquent.

User Stories
1. As an end user, I want to receive a notification via email that a new document(s) is ready for me to complete so that I can take action.
2. As an end user, I want to receive a second notification once too much time has passed to complete form(s) so that I can take action.
3. As a DMS user, I want to have the option to be copied on second notification to end user that their forms are now delinquent so that I can follow-up to ensure information is completed.
4. As an end user, I want to receive a copy of my completed form via email so that I have a copy of the completed form.
5. As a DMS user, I want to have the option to be copied on the completed form notification sent to end user so that I know when forms have been completed, and have a copy.
6. As a DMS user, I am able to set up my preferences via account settings page so that I receive the notifications I want.

Entry Criteria:
1. The end user opens notification email.
2. The DMS user opens notification email.

Exit Criteria:
*The end user clicks on button and is directed to portal to fill out form(s).
*The DMS user closes notification email.

Acceptance Criteria:
*IF I am an end user, I receive notifications via email so that I can take action.
*IF I am a DMS user, I have set up my account so that I receive notifications I desire to get.
*IF I am a DMS user, I receive email notifications reflective of my account settings.
