Create Document Template | Form fields are not refreshed when source product is changed.

Forms fields on Create Document Template are not refreshed when Source Product is changed.

1. Select CBES as Source Product
2. Mapped Address field
3. Changed the Source product to CBATS
4. The address field is removed from the mapped fields and is not populated under form fields.
