Description

Company - UiRefresh_DC_QA_13554

Server - Code Map

Step:-
1.Login to application using sa
2.Navigate to OnBoard > W4 download
3.Click on Add Document
4.Enter all details and click on Save

Actual - It did not create the report

Expected - it should save the details and redirect to manage page
