Kindly find below 3C's :
Concern : UI - Any company - EE Login - tour- spouse- blank fields
Cause : Field rights are not set properly in form builder.
Correction : Changed field rights.
NOTE :
Suppose i have field 'Employer Information' then it will contain its label name i.e 'Employer Information' and its containing fields like FirstName,LastName,Initial,Title,SSN,Gender.... etc..
If its contained all fields has right as 'No Rights' i.e its not visible to employee/company admin as per form builder setting then we have to set rights to its label as 'No Rights'..
Otherwise field label will visible to user.
Kindly do take care for these things while setting rights.

Verified on azure.
Field rights are set properly. No blank fields observed now on spouse and child page.
