Hi shyam sharma,

Code changes and script is done for this JIRA, but we can not deploy it on WTDEVDBSERVER as whole Workterra team is using this as DB server. We had requested new machine[10.0.15.18] which will be used for these changes and same machine will be used as DB server for UI refresh team. Today morning I've received access of this machine. After configuration changes, all UI refresh team members will point their DBserver to 10.0.15.18 and we will deploy these changes on that machine.

I have tried above scenario on EL Dorado HSPL and observed that one additional form not getting displayed on Employee Dashboard but same additional form getting displayed at EE workflow.

Hi [~meghana joshi],

I checked this from back-end and found out that menu for additional form "Your Enrollment Start Here" for Employee login was not active that's why it was not reflected on UI.

Tested below scenarios on Azure.

1) Verified Additional form are eligible or not by adding class eligibility criteria.
2) Verified Additional form are eligible or not by adding additional eligibility criteria
3) Verified Plan are eligible or not by adding additional form Fields eligibility criteria on Plan.
4) Verified Benefit types are eligible or not by adding additional form Fields eligibility criteria on Plans.
5) Verified Additional form are eligible or not by changing setting of Add Form in Menu options .

Above scenarios are working except ( 5 ) . Issue Logged for ( 5 ) under NF-3777
