Tested below scenarios on Azure.
1) Verified Additional form are eligible or not by adding class eligibility criteria.
2) Verified Additional form are eligible or not by adding additional eligibility criteria
3) Verified Plan are eligible or not by adding additional form Fields eligibility criteria on Plan.
4) Verified Benefit types are eligible or not by adding additional form Fields eligibility criteria on Plans.
5) Verified Additional form are eligible or not by changing setting of Add Form in Menu options .
Above scenarios are working except ( 5 ) . Issue Logged for ( 5 ) under
NF-3777CC - Sachin Hingole
