The new field Range Percentage of Monthly Salary is updated in Coverage Amount determined By
Start Percentage Value

Increment Factor
appear once the Range Multiple value is selected.
As all the new elements added are appearing as expected, moving jira on Stage.

We have added new option in the dropdown - Coverage Amounts are Determined By
i.e. Range Percentage of Monthly Salary
with fields Start Percentage Value, End Percentage Value and Allow Salary Displayed On Enroll Now Screen
Corresponding business logic jira is NF-198.
