Form Library - Add Form
1) Grid is not properly displayed and show entries option is not properly displayed
2) Selected form library category align is not proper and font size of navigation are too big
3) Nothing happens on clicking Browse / upload , Add button
4) UI of Add Form are completely disturb

-For the third point I have added the bug NF-697, and the 4th point issue is still exists, hence reopening this issue.
- First and second point seems fixed.
