      In SA login, while managing employee, In 'My Carrier Contact Information' widget, if any 'Benefit Provider' selected, then the page loads with path containing text 'Summary Page ' instead of 'Benefit Provider'. PFA screen shot.

            Verified on Azure using company UiRefresh QA Test1 in SA login. Above issue is resolved now.
