All mentioned fields are mandatory on Benefit Type Builder page,so they are blocked to Select/deselect checkbox.

But Benefit Provider is showing with black check mark as shown in below attachment : -

Employee Enrollment Permissions,Is Cobra Plan?,Plan Type,Plan Name is showing white check marks as shown in below attachment : -
