      Description

      Credentials Used:
      Account: User ID: 301699
      Password: New!Pass@2
      Issue:
      On the Enroll Now medical page, there is verbiage for adding the primary care physician (see screen print attached). This box never appeared.

      How it should work:
      A box should pop up allowing a number to be entered and providing noted links.
