      Description

      Please follow below steps to repro issue:-
      1.Login With SA / Admin.
      2.Go to Performance management.
      3.Click on Recognition
      4.click on New button
      5.Insert some data and save.
      Actual result-It is not showing any Information for some mandatory fields (Type and Icon)
      Expected result- It should show Information for mandatory fields and also after saving information
