Steps:
Issue#1
1. Partner Login > Benefit Description > Add form > Add more than 10 form (example 12 ) forms are added
2. Benefit Description > Select any Plan > Add form > Select All 1 form > Save
3. Form Library > View form > last two forms are seen unchecked
Expected Result: Form no. 11 and 12 should have disable checkbox as they are added in Benefit Description.

Steps:
1. Form 11 and 12 in above steps when selected to delete gives server error
