Pre prodction- IPR for hspl - Classwise OE - Affected template - UI is not proper and buttons are not working

Login as partner
Company information->Class wise Open Enrollment Effective Date Rules
Make changes in rule and click on update button.

Actual result-Affected template popup opens.
Following issues observed:

1. UI is not proper.
2. Not able to click on save and cancel button.
3. Not able to select checkbox
   Expected result: UI should be proper.all buttons functionality should be works properly.
