      Description

      Tested on Mozilla 52, Chrome 52, IE 11 with UIRefresh company

      Steps:-
      1. Add start date and end date
      2. Upload the form
      3. select some eligibility criteria
      4. Select the determine eligibility using zipcode
      5. Click on save button

      Expected:- Form should get submitted

      Actual:- Server alert is getting display.
