Steps:-
1. Login as Admin
2. Go to format confirmation statement page
3. Select all the pages and click on the save button
4. Login with the new employee
5. Perform the self serve mode
6. Go to the confirmation page
Expected:- All the pages should get display.
Actual:Only few pages getting display, PFA the screen shot.
Please see the attached screen shots, of stage as well.

I have verified the issue.
Environment:Azure
Browsers: Chrome, IE-11
Verified with SA
Expected:- All the pages should get display.
I am not able to perform self serve mode (its not going beyond welcome Page)
