We have imported code correction for optimum.
We have rerun the 'OPTIMUM 1095c Report' by setting 'Apply User Correction' flag to 'YES'.
Please refer latest run '2/21/2018 3:37:30 AM' to download.

Tested scenarios/ observations are as follows

1.MEC indicator is marked as 'NO' for all 12 months.
2.Checked all Valid codes are reflecting on report.
3.Discrepancy Report displayed discrepancy in Street Address 1 and State of few employees.
