            Jennifer Leugers Please see below feedback from IPR. I will update the company address in WORKTERRA. The broker will be sending us a COBRA report of those who need forms since they do not house this info in WT. Also, I have saved their correction template to the Y drive in their ACA folder.

            Hi Ashley,

            The “2017 ACA” folder on the FTP site is empty. I found the files under “Reports” on the FTP site. It’s titled “drafts 12.20.17 1095C_fir_IPR.zip” and “drafts 12.20.17 ACA 1095C Analysis.xls”. I was working off these files tonight.

            ACA 1095C Codes
            · For Column R, it’s understood that employees that were terminated or not employed with IPR would have a “part time” status?
            · Rows 1763 through 1774 is Delete Delete – is this a typo for employee name? SSN: 264-87-3120
            · Minimum Employee Cost Offered (column U) and Minimum Employer Cost Offered (column V) – I see several employees that were not eligible for coverage. Are we still required to provide them forms? Some examples are below:
            · Juan A. Aguilar Gonzalez – SSN: XXX-XX-8571 – he was hired and terminated the same day
            · Philip B. Chandler – SSN: XXX-XX-2389 – hired and terminated in same month, so no coverage offered at all
            · Christopher A. Sanchez – SSN: XXX-XX-5382 – hired and terminated in same month, so no coverage offered at all

            Would like to confirm that there are no records for this, so nothing I need to review over, correct?

            On the ACA 1095C Correction Import Template, I’m not sure what the “Plan Start Month” is and have left that blank.

            I entered for:
            · Brian Gildo (SSN: XXX-XX-7838) – he had union medical coverage for the month of April, starting on 04/04/2017
            · Charone Green (SSN: XXX-XX-8506) – he should be on waiting period for month of August
            · Jeremy Pilkinton (SSN: XXX-XX-1255) – he should be on waiting period for month of September
            · Mark Radvansky (SSN: XXX-XX-5840) – his date of hire had been changed and he should not be on the waiting period in October since he was hired on 11/19/2017
            · Eduardo Salinas (SSN: XXX-XX-9364) – terminated in November, so should have a code update for December
            · Michael Martinez (SSN: XXX-XX-9794) terminated in November, so should have a code update for December
            · Adophis Jackson (SSN: XXX-XX-9492) declined coverage in January when he became eligible and terminated 01/30/2017, so should change code to be full time

            Just want to confirm that for all that had code 2B on Line 16 – they were terminated in that corresponding month and did not have coverage?

            COBRA participants are not in WT. Stephanie/Vanessa, I don’t recall how we provided the COBRA list to WT last year – maybe it was the provider or did I pull from CONEXIS/Wageworks?

            The company address on each 1095C form needs to be updated to the following, please:

            The Woodlands, TX 77381

            All returned mail should go back to me at that updated address above.
