Hi all - please see comments from the Client below and advise. They are also stating that they have 106 Retirees that are missing. I've attached the list herein.
We have finished spot-checking a portion of the active employees (and employees who retired mid-way through the year) in each plan type, and we noted the following items/questions:
Jones, Larry J.– Spouse does not have valid SS# (000000000). It looks like the employee never provided this to us. I have left him a voicemail to see if he could provide this information. If he does not get back in time, would it be possible to list the spouse’s date of birth instead?
Dillon, Carla D.– This is one of the only ones where we saw the code “1E” in Box 14, and we wanted to make sure what this means, and whether this is accurate.
Ortega, Robert L.– This is one of the only ones where we saw the code “1E” in Box 14, and we wanted to make sure what this means, and whether this is accurate.
There were a few employees who retired part-way through the year that have the box checked for coverage for all 12 months (and were indeed covered on a combination of employee and retiree coverage for those twelve months), but the months after their retirement have the code “1H” in box 14. Is this correct? The names who came up in our spot-check included James Davidson, Roy English, Alvin Palmquist, Wendy Sevenandt, and Kimberly Christensen.

We have verified employee 'Aegerter Wayne' from 'Updated-OCSD_missing' document and observed that employee is enrolled in 'Retiree Kaiser $15 HMO Traditional' from 01/01/2017 but actually this plan is not marked as ACA plan hence this employee's form is not generatred through system.Please reconfirm plans on ACA customization screen under Retiree Medical.

+Harbinger Comment: +We can not change Employee data, even though if you need to do so. Please raise a qualify event and remove spouse SSN from spouse demographics page then system will automatically consider 'Spouse’s date of birth' in next run.

Harbinger Comment In case of Cobra/Retired employees, according to '2017 Instructions for Forms 1094-C and 1095-C'
Line 14 – Always use Code 1H (no offer of coverage).
Line 15 – Leave Line 15 blank.
Line 16 – use Code 2A (employee not employed during the month).
Part III – Individual Monthly Coverage Information. Employers with self-insured plans will complete this section for former employees and dependents who enroll in COBRA and will continue to report for as long as COBRA coverage is in force.
We have verified employee 'James Davidson' at our end.

Please find my comments below two employees:
Dillon, Carla D : Employee was terminated in September. Employee had offer of Kaiser Bronze plan which has lowest cost.As he was terminated in September,this offer also got terminated. He was enrolled in Anthem PPO plan till 9/30/2017,this plan is the only plan for which he had offer for whole of September month. As it's cost is $167.6 ,1E is shown for that month. This is accurate.
Ortega, Robert L.: Employee status was changed from Inactive to LOA in June. So for some days in June month, employee did not had offer of lowest plan "Kaiser HMO". As he was enrolled in Anthem PPO plan during June Month,1E is shown as per it's cost. He was changed from LOA to Active effective 06/30/2017,he became eligible for Kaiser HMO from July.Hence the codes displayed are accurate.

We have gone through to check the issues and/or questions that we had noted during our spot checking, and some of the items are still present. I have summarized any remaining items in the attached spreadsheet.
In addition to the items we had noted in the first review, I noticed the tab on EBS’ analysis spreadsheet listing a few dependents with abnormal social security numbers, and I have added these to our list as well. We had already noticed Larry Jones’ wife in our spot-checking, but the other five were not included in our original list. For these dependents, would it be an option to list their DOB rather than the social security number?

On verification of ACA Customization,we found that below plans were not marked as ACA:
2015 HMO Premier 15 w/Chiro Traditional
Retiree Kaiser $15 HMO Traditional
Retiree Kaiser Sr. Advantage
We have marked those as ACA and MEC plan and re-run the forms.
1. The retirees are not pulling in the forms is because they are enrolled in a plan that is not self insured.
Forms for Retiree/COBRA will be generated only if they are enrolled in self insured plans.
2. In the attached excel,there is a comment for EE Baxter Anthony on whether forms can be consolidate as spouse also has form.Forms are generated for each and every eligible employee of the company.So they cannot be consolidated.
3. Please find my earlier comment for the clarification for the Employees : Dillon, Carla D and Ortega, Robert L
4. For the EE's who have retired part way,1H/2A will be displayed for the months they were not employed.But Part 3 will be marked.

5. For invalid SSN, DOB will automatically be populated if invalid SSN are removed from the System by raising qualifying event. DEV team cannot do changes to the system.

HI all - changed two employees to 1H/2A for all 12 months but they keep defaulting back to 1G when I run the forms. Please advise as to why (they are the only two in the corrections tab for this client).

These employees defaulted back to 1G even after code correction as priority for code 1G is higher.We have changed this and re-run the report.
Please refer latest run for verification. EE count is 2 Less than the previous count.
On comparing with last run,the only change was the two employees who got deleted as their code was corrected to 1H/2A for all 12 months.
