Hi Debbie Kulling,

The field selected has character more than 128 and report service tool has limitation of 128 characters.

If we try to extend beyond 128 characters system throws error. So we trim the names beyond 128 characters in the code logic.

Reason of "SPCHAR511134" appended in header name : 

If two or more fields have similar name up-to 128 characters, then on trimming all will become same name and system will throw error again. So internally we append some random codes that distinguish such field names with each other.

If those fields are really required to be shown with complete name, then we can do more R&D if this is anyhow possible. But we are not sure on this.

We cannot format/modify radio group label in above mentioned way. 

But, we can add a paragraph control and add formatted text in that. Below paragraph control , we can place radio group control below paragraph. This can be done by editing form from website.

We have changed AFLAC Certification New --> CRITIAL ILLNESS CERTIFICATION field as per above mock-up. It worked as expected and It made no impact on any existing eligibility.
