We are able to reproduce this issue from CHMC (Prime) please refer following steps:-
Company Name:- CHMC for HSPL
Employee:- BEHNAZ KARIMIAN
Plan Name:- 1X Annual Salary to $50000 max Centinela
Rate Name:- 2017 Basic Life and ADD
Step 1:- Delete Existing Duplicate election rates(ID=196542,196534)
Step 2:- Login into production with partner user.
Step 3:- Click on Change Status for Employee and change Reason for Status Change and Status Change Date to '08/18/2017'.
Step 4:- Check Duplicate Rates in Database.
