Configure "Affidavit Form " For some Plan

1. Login with Employee ( Joshua. Admns | Password@1 | For qa asml for hspl)
2. Got to Enroll Now Page
3.Select Plan for which " Affidavit is Configured "
4.Click on view Affidavit
