Hi Jackie Sivigny,
We have almost done with the document provided for AFFCO Company Information
Following are our two queries:
1. As per the boss document in >> Payroll schedules “Semi-Monthly” and “Monthly” Only “Pay Dates” is given in excel sheet.
Please provide “Pay Periods”(Start and End date)
As of now we have kept the system generated “Pay periods” as it is.
Please check and verify and let us know your thoughts on this.
2. In company information >> Company Contact Information >> for “Charlie Kirchhoff” mandatory field “Email” and “Phone Number” is not provided. Request you to please provide these details. Thank you.
------------------------------------------------------------------------------------
CC: Vijay Siddha, Satya, Mandar Kulkarni, Venkatesh Pujari, Jaideep Vinchurkar, Prajakta Belhe, Amanulla Mulla
1. Pay dates are the only fields that should be entered. Gray fields should be ignored in BOSS and in site. Ignore Pay Period Start and End Dates in the site.
2. I have attached a revised BOSS with the contact info for Charlie
Hello Venkatesh Pujari,
Could you please look in to this? Thanks.
--------------------------------------------------------------------------------------------------------------
CC: Vijay Siddha, Satya, Jaideep Vinchurkar, Sumeet.Kolge, Amanulla Mulla, Prajakta Belhe
Hi Jackie Sivigny,
Thank You for the quick response.
We have updated the contact the details for Charlie. But please note that as per your first point i.e ignore pay period start and end date this is not possible. Because in the current system Pay Period Start and End Dates cannot be blank. Let us know what should be done for both the payroll schedules ?
Thanks,
Venkatesh
sorry I should have been more clear. No adjustment needs to be made to the pay start and end dates. They can remain as the system generated dates. Let me know if there is anything else.
Hi Jackie Sivigny,
While editing the payroll schedules I have observed that the Pay Dates that have been provided are very less as compared to the pay periods generated by the System. Please let us know what should be done for both the payroll schedules.
Thanks,
Venkatesh
Hello - I have attached the Plans & Rates BOSS for build as well. I hid the Life and User Defined plan tabs as I still need to audit. I will attach a new P&R version with those tabs exposed as soon as audited. Please target to have these done by 10/10.
Any questions/concerns let me know.
thanks,
Jackie
Hi Mandar Kulkarni,
Due to time constraint I was not to work on this. Please could you take this forward as Jackie needs this to be done by 10/10/2017.
Thanks,
Venkatesh
Hello Jackie Sivigny
As Venkatesh Pujari mention above, the Pay Dates are provided in the sheet for "Semi-Monthly" are from 01/10/2017 To 12/25/2018
And for “Monthly” is from 01/31/2017 to 12/31/2017
However the system generated pay dates are more than it is provided in the excel sheet.
(Refer below screen shot)
Please let us know your thoughts on this. Thank you!

CC: Mandar Kulkarni, Venkatesh Pujari, Vijay Siddha, Prajakta Belhe, Amanulla Mulla
Thanks Sumeet! Correct - you can ignore the dates above and beyond the dates outlined in the BOSS. In subsequent years, we will update those as pay schedules are provided by Client. Anything else let us know. Thanks!
Hello Jackie Sivigny,
We are almost done with the "Plans and Rates" (Benefit Type: Provider, Health Tier labels, Health Plans ) document, we will have a round of review tomorrow.
Here is a query we have:
As per the document in health plans >> Plans 8 to plan 21 >> under cost and coverage >> Cost Deduction Schedule is provided in the sheet as "Monthly".
However as per the document we have following payroll schedules in the system:
- Monthly-Cobra
- Semi-Monthly
As of now we have selected as "Monthly-Cobra", let us know if our understanding is correct.
Please let us know your thoughts on this. Thank you.
---------------------------------------------------------------------------------------------------------
CC: Mandar Kulkarni, Vijay Siddha, Jaideep Vinchurkar, Venkatesh Pujari, Amanulla Mulla, Prajakta Belhe
Hello,
You are absolutely correct in your selection. For the plans identified as Monthly it should be aligned to Monthly COBRA. Sorry I should have caught that in my review.
thanks,
Jackie
Hello,
I have attached another Plans & Rates BOSS that is titled "AFFCO - 2 - Plans and Rates BOSS 1009" - this contains all of the remaining plans requiring your build. Additionally, I have attached the EE Self Service BOSS for update as well. I will attach the remaining 3 BOSS documents once audit and/or build out is complete.
Please target having these done by EOB 10/11.
Thanks,
Jackie
Hello,
I just attached the PDHT mapping BOSS as well. Please target having that completed as well by EOB 10/11. Any questions/concerns let me know.
Thanks again,
Jackie
Hello All,
Sumeet.Kolge, Venkatesh Pujari, Amanulla Mulla, Prajakta Belhe, Arpita Verma, Shaikh Ateeque, Satya,
Let's take this up today.
----------------------------------------------------
CC: Vijay Siddha, Satya, Jaideep Vinchurkar
Hello Jackie Sivigny,
We are done with the first step of implementing all the documents provided by you (life and PDHP)
Here are the queries we have:
1. As per the PDHP Document >> the followings sheets named in the document:
ACTIVE COBRA BCBS
COBRA BCBS
BCBS
ACTIVE COBRA VISION
ACTIVE COBRA Dental DHMO
Active COBRA Dental PPO
Active COBRA Medical5KPPO
ActiveCOBRAMedical1KPPO
Active COBRA Medical2.5KPPO
We would like to know, in which plan the above health tiers should be mapped?
2. In Company Information >> EDS
Enroll Now- Deductible Buffer
Enroll Now- Universal Life
These fields are not provided in benefit types as per document "Plans and Rates", hence we have skipped these fields.
3. In life plan >> plan no 2 >> "Hourly Basic AD&D" the plan type is given in sheet under "Basic Life and AD&D" benefit type.
However there is no such benefit type as per the document provided, as of now as per our understanding we have entered the plan "Hourly Basic AD&D" in benefit type "Basic Employee AD&D".
Please let us know if our understanding is correct.
4. In life plan 7,9,11 >> as there is no PDF document "Lincoln EOI" is provided, hence we have left this field.
Please let us know your thought on this.

You guys are great! See below.
1. The only tiers that should exist in the site are EE, EE+SP, EE+CH, EE+2CH, EE+SP+CH and EE+SP+2CH. Those are the only ones that should also have values attached in PDHT mapping.
2. That is fine. We can add this later. We are still in process of determining if needed as User Defined or Life.
3. Your understanding is accurate.
4. I apologize I added yesterday but must not have saved. I just added and double checked that it saved. If you could attach the form to the plans identified that would be great!
Anything else let me know.
Thanks,
Jackie
Hi Jackie Sivigny,
The only tiers that exists in the site are EE, EE+SP, EE+CH, EE+2CH, EE+SP+CH and EE+SP+2CH.
However, below is our concern:
In customizer >> benefit library >> plan design health tier mapping document>> sheet name
1.ActiveCOBRAMedical 1 KPPO
(Refer screen shot)
2.Active COBRA Medical2.5KPPO
3.Active COBRA Medical5KPPO
4.Active COBRA Dental PPO
5.ACTIVE COBRA Dental DHMO
6.ACTIVE COBRA VISION
7.BCBS
8.COBRA BCBS
9.ACTIVE COBRA BCBS
In which plans we have to add these plan design health tier mapping?
Please let us know your thoughts, Thank you!
------------------------------------------------------------------------------------------------------
CC: Mandar Kulkarni, Satya,Vijay Siddha, Venkatesh Pujari, Prasad Patil
Sorry about that am following now. See below.
Active COBRA Medical 1k PPO - should be Active Medical 1000 PPO
Active COBRA Medical 2.5k PPO - should be Active Medical 2500 PPO
Active COBRA Dental PPO - should be Active Dental PPO
Active COBRA Dental DHMO - should be Active Dental DHMO
Active COBRA Vision - should be Active Vision
In the BCBS tab it is plan BCBS Medical PPO1K - should be BCBS Medical PPO 2000
COBRA BCBS - should be COBRA Medical PPO 2000
Acitve COBRA BCBS should be Active Medical PPO 2000
Hello,
I just attached the Rates BOSS and Other Core Build BOSS to this ticket. Can we have this final part of build completed by 10/12? If you could complete by EOB 10/11 it would be amazing but realize that is a short TAT.
Let us know once build is complete and QAT is done.
Any questions let us know.
Thanks,
Jackie

Query 2:
In ACA Parameters there is a field 'Safe Harbor Cost In %' for which the value is not mentioned in the excel sheet. We have left the value for that field as it is as shown in the figure.

Hello Jackie Sivigny
We have reviewed all documents, during our review we found one query below :
In Life plans as per the document "Plans and rates", 'Manager Basic Life AD&D' Plan is showing under 'Basic Employee Life' Benefit type.
But in 'AFFCO 2A Rate Tool New ' document the Rate "Basic AD&D" is mapped to 'Manager Basic AD&D'.
Please confirm whether 'Manager Basic Life AD&D' comes under 'Basic Employee Life' or 'Basic Employee Life AD&D'
Please let us know your thougts on this. Thank you!
