It looks like when the copy from Prod to stage was completed, there was an error with the additional eligibility for the following plans: (2018 are new but cloned)
2018 Tobacco Employee and Spouse Surcharge 01/01/2018 0 Rate Mapped
2018 Tobacco Employee and Spouse Surcharge Industrial 01/01/2018 0 Rate Mapped
2018 Tobacco Surcharge Industrial 01/01/2018 0 Rate Mapped
2018 Tobacco Surcharge Plan 01/01/2018 0 Rate Mapped
Tobacco Surcharge Industrial 01/01/2017 - 12/31/2017 2 Rates Mapped
Tobacco Surcharge Plan 01/01/2017 - 12/31/2017 2 Rates Mapped
The form " Tobacco Affidavit Form" (Renamed from Tobacco Affidavit 2017) is showing up as
"Preferred Email" - This was there even before we changed the form name.
Can you please look into this ASAP?

Concern:-
Austin - Additional Eligibility.
Cause:-
While updating page field configuration, if base field and additional field id's are same, additional field display label was getting updated.
Correction:-
We have handled this while updating page field configuration.
Impacted Areas:- Eligibility on Fields.
Affected files:- OES_TR_FIELDS_AFT_UPD.Trigger.sql, OES_TR_FIELDS_AFT_DEL.Trigger.sql
Note: There was also issue with delete trigger, so made changes accordingly in delete trigger.

1]Add tow three Fileds on an additional form
2]additonal eligibility criteria used above fields.
3]Change Fileds name on the additional form.
4]Cross verified on eligibility criteria.
5]Clone the filed and cross verified on eligibility criteria.
6]not showing any duplicate filed name
working fine as expected
Verified on production consider below scenarios:
