Hello,
The CE is working on a custom form for my client Gordon Flesch and after she drag and drop the drop down box over and she typed in the question and hit save I get these error messages.
I'm working on their site for OE.
The name of the form should be: 2018 HRA
the field name should be : Did employee complete HRA for current year?
and the drop down list should consist of: Please make a selection, Yes, No

Please find below 3C's of the issue,
Concern:Error Message when trying to build a custom form
Cause: We found that,there are duplicate generic eligibility rules.('All Status Rule').
That were causing issue while inserting data of newly added control.
Correction:We have renamed one of the rule to 'all status rule duplicate'.
and also corrected the field entries.Now you can add the control.
Please verify and also delete the duplicate rule as it is not in use.['all status rule duplicate'.]
