          Can you please send the ACA forms to the client. The data for 16 is not populating because the employee does not have a waive or make an enrollment.

          Not all corrections were made - see below.
          Also, it appears that the 30 hour/week filter is not being applied because part-timers are appearing on the list and have a 1095C.
          Do you need that "sign-off" form marked with "no", signed and returned...

          There are two issues which affect every form.

          1.We wanted the affordability to be calculated based on rate of pay and not federal poverty line. The form shows box 14 with code 1A but should be 1E. CORRECTED FOR 1ST 6 MOS ONLY.
          2.The Plan Start Month should be 7 (July) rather than 1 (January) NOT CORRECTED

          Specific issues with employee forms include the following:

          1. Colleen Lavery was offered insurance but waived it. The code for box 14 should be 1E and box 16 should be 2H ISSUE WITH JULY-DECEMBER - SEE #1 ABOVE

          2. Alan Owens is a Public Works Employee. Box 14 is correct but box 16 is blank and should be code 2E NOT CORRECTED - 2E MISSING

          3. Aaron Gerdes is a firefighter who had insurance for all twelve months. The 1095 is showing only 6 months. BOX 16 INCONSISTENT - BOX 14 SHOULD BE 1E

          We need these issues resolved today. We feel we are in jeopardy of missing this deadline.
