Hello,
Prime Healtcare advised that employee Susan J. Horvat's history shows Enrollment Terminated and Enrollment added by "EDI Test." Screenshot attached: 
Prime Healthcare is asking that WorkTerra run reporting showing any entries made by "EDI Test" that changed plan terminations from 2017 to a December 31, 2016 date. This needs to be done for all Prime Healthcare facilities:

- EHMC

- Hospitality

- MHIN
- MRTX

- Prime Healthcare
- Primehealthcare
- SRMC
For this reporting, we require the following fields:
- Facility
- Last Name
- First Name
- SSN
- Classification
- Plan Name
- Enrollment Date
- Termination Date
This reporting needs to be created at your earliest convenience.

We have scanned records affected by "EDI Test" and "DevImport" users for given Prime Healthcare facilities. Please find attached document WT-12813_Report.xls
for more details. I will share password in separate mail.
We have provided requested details like - Facility, Last Name, First Name, SSN, Classification, Plan Name, Enrollment Date, Termination Date which was present in system before "EDI Test" and "DevImport" users made changes and additional column for termination date set by these users.
